Door het selectievakje aan te vinken verklaar ik:

de bijpassende toelichting te hebben gelezen;

voor eigen rekening en risico activiteit en/of studieonderdeel in het buitenland te volgen;

hiervoor wel goedkeuring van het LUMC te hebben gekregen (indien nodig);

in te stemmen met de mogelijkheid om binnen de komende twee kalenderjaren contact met mij op te nemen over mijn buitenlandervaringen (door LUMC medewerker en/of student);

dat ik me aan de gedragscode van het LUMC houd;

alle mogelijke voorzorgsmaatregelen te hebben genomen om gezondheidsrisico’s te vermijden;

mij voldoende te hebben verzekerd en het LUMC in geen geval aansprakelijk te stellen voor welke schade dan ook.
